The Individual Support Plan (or ISP) has been co-produced and developed with West Sussex schools. The Pilot began in 2021 with the Burgess Hill Locality and AIIB (area inclusion and improvement board). The project focused on developing a SEN support / additional needs template which aligned with the Ordinarily Available Inclusive Practice document (which was also co-produced by West Sussex Schools and promoted early intervention and adaptive teaching practice within the classroom).

Aims included:

  • To support the consistency of quality first teaching and SEND practice.
  • To support consistent implementation of the graduated response.
  • To facilitate joint working between class teachers/key workers and SENDCo’s.
  • To improve the evidence base for referral request
  • To provide a consistent approach and offer for parents
  • To promote positive educational outcomes for children and young people within the Burgess Hill Locality.

The template has been piloted across a wider range of schools and settings in the 2022 to 23 academic year with very positive feedback and results. In consultation with Schools, Inclusion Leads and the Advisory Teams, the ISP is therefore being rolled out across the county during the 2023 to 2024 academic year.

The ISP:

  • Is free of charge to access and use via West Sussex Tools for Schools
  • Supports information gathering at the earliest stages of support
  • Scaffolds the graduated response and assess, plan, do, review cycle for non-SEND specialists, making every teacher a teacher of SEND
  • Promotes co-production with children, young people, and their families
  • Digitises record-keeping for easy access and co-productive updates
  • Supports effective transitions and information-sharing
  • Is co-produced through an extensive pilot project, aligning with the Ordinarily Available Inclusive Practice guidance
